Dr. Kay performs consultations and records reviews as a service to her clients. However, Dr. Kay receives dozens of requests for advice and help every day and cannot offer recommendations for evaluation, educational programming or placement until she has reviewed the child's file. Before a Consultation Before requesting a consultation, you should read the articles at this website, especially:
To Obtain a Consultation Step 1. Complete an Intake
Questionnaire; sign and date the Fee Policy and consent; have
teachers complete Teacher Input Questionnaires; and
complete the
Parent Input Questionnaire.
Step 2. Organize your file with records in chronological order (oldest documents
on top). Step 3. In the lower right hand corner on the first page of each document, pencil in the date of the document (i.e. 10/13/99) Do not use a pen---the date may need to be erased later!

Telephone Consultation Option The consultation fee includes the time taken to review the file and conduct any educational research and the telephone consultation itself, which lasts approximately 30 minutes. Telephone consultations must be scheduled beforehand by you with my office. To schedule the telephone consultation, please call (717) 569-6223 and ask to speak with Kathy. She will schedule a one-half hour consultation session with Dr. Kay to discuss submitted records and to provide verbal recommendations. The telephone consultation must be initiated by you at your scheduled time. Failure to initiate the consultation will result in a forfeiture of the consultation fee. Rescheduling of consultations must occur at least 24 hours before the appointed time to avoid forfeiture of the consultation fee. Feel free to tape the telephone consultation. Written Report Option The consultation fee includes time to review the file and conduct any educational research and reparation of a written report which contains Dr. Kay's recommendations for further testing (if needed), intervention and educational programming based upon submitted records. The written report will be sent to you within 15 working days after the educational records have been received. If you also wish to have a telephone consultation after reviewing the written report, please call (717) 569-6223 and ask to speak with Kathy. She will schedule a telephone consultation with Dr. Kay to discuss submitted records and the written report. Additional costs for follow-up telephone consultations will be billed at a rate of $150/hour. Fees must be pre-paid by check, money order or credit card prior to a scheduled consultation.
