Professional Consultations

Dr. Kay performs consultations and records reviews as a service to her clients. However, Dr. Kay receives dozens of requests for advice every day and cannot offer recommendations for evaluation, educational programming or placement until she has reviewed the student's entire file.

To Obtain a Consultation:

  1. See Client Forms section of this site
  2. Complete either a Child Intake Questionnaire or an Adult Intake Questionnaire.
  3. Sign and date the Fee Policy and Consent
  4. Have teachers complete Teacher Input Questionnaires
  5. Complete the Parent Input Questionnaire
  6. Organize your file with records in chronological order (oldest documents on top)
  7. DO NOT SEPARATE DOCUMENTS BY CATEGORY (i.e. IEP's, report cards, evaluations,medical records, etc.)
  8. In the lower right hand corner on the first page of each document, pencil in the date of the document (i.e. 1/1/13) Do not use a pen---the date may need to be erased later!
  9. Do not write notes on the documents
  10. Do not highlight information
  11. Put completed questionnaires on top
  12. Send photocopies not originals
  13. Limit the physical size of the file to two (2) pounds or less.
  14. Send the completed intake form, parent and teacher questionnaires, file and check in the amount of $1000.00 (US currency) to -
